A large number of ambulatory services are private. When's the last time you saw an ambulance driver wait for your credit card to be approved before driving you to the hospital?
Except in your case they know damn well they're getting paid. I've been picked up by ambulance, and there's not much you can do after the fact besides get collectors on your ass.
