A wonderful scene empowered even more when Madsen steps out to the car. And knocks the audience out of that place into something totally serene with birds chirping...

I recall listening to some commentary somewhere where it was remarked that it feels even more gruesome b/c they don't actually show it - your mind makes it sound even 'grosser' than it would be in reality.

It's called the Roman Polanski method of filming, since probably the first and best example of such a technique was Rosemary's baby.

The audience never sees the baby, but we see the terrified expression of Mia Farrow as she frantically questions what they have done to her baby. Our imaginations each contort a image of the baby that is subjectively horrifying to us.
