Ironic answer: Irrational fear of slides
^ Not to burst your bubble, but the only way he could get out to help people down the slides was through his window. The crush of bodies probably jammed the cockpit door solid.
Or the config of the emergency door, not just the bodies. Still I like "fear of slides"
"no fire" because they no doubt ran a bunch of laps until the gas tanks were almost empty!
