The reasoning is that it's supposedly insurance. If you're looking to get fire insurance for your house, it's probably fair for the insurance company to refuse to cover you if your house has already burned down.

If there was a law preventing them from doing that, then there would be no incentive to purchase insurance when you're healthy since you can just wait and buy it when you get sick. That's why the bulk of the proposals before congress really do two basic things; ban preexisting conditions, and require everyone purchase insurance.

The problem is that as things are now, the insurance companies seem to have a completely free hand to determine what's considered a "preexisting condition." There's also a ton of profit incentive for them to make them as broad and far-reaching as possible. After all, their goal is to collect premiums from people who're healthy for extended periods of time, and pay for the smallest number of people's care as they can -- that's how they make a profit, after all.
